Membership Assistance Program (MAP) Forms

This page contains Spending Plan and Follow-up Report forms for the Membership Assistance Program (MAP).  The forms are in pdf format and can be used in two ways:

1. Print the forms and complete by hand or typewriter, then sign and mail or fax to the SFA Office, 510 Cynthia St., Saskatoon  SK S7L 7K7, (306) 242-8007 (fax).
   • MAP Spending Plan (MAP application) for completion by hand
   • MAP Follow-up Report for completion by hand
      
      
2. Complete the forms online, then print, sign, and mail or fax to the SFA Office, 510 Cynthia St., Saskatoon  SK S7L 7K7, (306) 242-8007 (fax). (Please note that you must still print and sign the forms for submission by mail or fax, they cannot be submitted electronically.)
   • MAP Spending Plan (MAP application) for online (electronic) completion
   • MAP Follow-up Report for online (electronic) completion
      

NOTE: In addition to printing and/or completing the forms through your browser, you can download them to your computer for use offline.  To download a form, right-click on its link and select 'Save Target As...' or 'Save Link As...', then choose a suitable folder.  (Depending on your version of Acrobat Reader, there may be a Save icon to also allow downloading the form to your computer.)
