Please note: Although it is most unlikely that you will experience any problems responding to this form, if you do experience any difficulties you may email your response to this form to: saskfencing@shaw.ca. In your message please be sure to include at least your name, Birth Year, CFF passport number, and your goal (goals) for the coming season.

Submitting this form registers your application for the 2006-07 SFA Training Squad.

Parental/Guardian Approval:
Please note that fencers under 18 years of age must indicate parental or guardian approval of their application.

Financial Commitment:
Also, please note that acceptance into the Provincial Squad Program may involve a significant financial commitment by you.  Prior to any trips, camps, competitions, etc. you will be responsible for all associated costs.  Any SFA funding to which you are entitled will be provided after the event, upon submission by you of appropriate receipts.
